पाइल्स (Piles) हा एक सर्वसामान्य आजार आहे जो कोणालाही होऊ शकतो. याला वैद्यकीय भाषेत मूळव्याध असेही म्हणतात. मूळव्याध म्हणजे गुदद्वाराजवळ(anus) असलेला कोंब. मूळव्याध हे दोन प्रकारचे असतात. पहिला आतील मूळव्याध जो गुदाशयाच्या आतमध्ये विकसित होतो आणि दुसरा बाहेरील मूळव्याध जो बाहेर म्हणजे गुदाशयाच्या भोवतीच्या त्वचेखाली विकसित होतो. मोठ्या झालेल्या रक्तवाहिन्या म्हणजेच मूळव्याधीचा कोंब(Hemorrhoids sprout) असतो. बहुतेक वेळा मूळव्याधीचा त्रास कमी असतो आणि रुग्णाला काही लक्षणे जाणवत नाहीत.

मुळव्याधाची लक्षणे जाणवत असल्यास बहुतेकदा ती खालील स्वरूपाची असतात(If the symptoms of Piles disease are felt, they are usually of the following form in Marathi) –

शौचानंतर रक्तस्त्राव – हे रक्त लाल रंगाचे असते.

• आहारातील Fiber पदार्थांचे प्रमाण हळूहळू वाढवणे – फळे,हिरवी पालेभाज्या, कोशिंबिरी, कडधान्ये, सुकामेवा इ. पदार्थ Fiber पदार्थांचे स्रोत आहेत.
• भरपूर प्रमाणात द्रवपदार्थ, भरपूर पाणी प्यावे. अतिरिक्त प्रमाणात चहा व कॉफीचे सेवन करू नये. मद्यपान टाळावे.
• शौचाची भावना झाल्यानंतर शौचास जाण्यास उशीर करायला नाही पाहिजे. असे केल्यामुळे शौच कठीण आणि कोरडे होते त्यामुळे शौचास जोर करावा लागू शकतो.
• काही औषधांमुळे (उदा: कोडीन वेदनाशामक औषधे) Constipation होतो. अशी औषधे घेणे शक्यतो टाळावे.
• वजन नियंत्रणात असायला पाहिजे.
• नियमित व्यायाम– यामुळे Constipation टाळण्यास मदत होते, रक्तदाब कमी होतो आणि वजन कमी होण्यास मदत होते.

या बदलांमुळे मूळव्याध होण्याचा किंवा पुन्हापुन्हा होण्याचा धोका कमी असतो. आपल्या डॉक्टरांनी सांगितलेले औषध गुदद्वाराजवळ आणि आत लावल्यास आपल्या लक्षणांची तीव्रता कमी होण्यास मदत होते तसेच शौच करणे सुलभ होते. .

मूळव्याधीचे निदान(Diagnosis of piles in marathi):

आपल्या गुदाशयाची तपासणी करून डॉक्टर मूळव्याधीचे निदान करतात.

गुदाशयाची व गुदद्वाराची तपासणी:

प्रथम डॉक्टर आपल्या गुदद्वाराच्या बाह्य बाजूस काही मूळव्याधीचे कोंब आहेत हे तपासतात. त्यानंतर अंतर्गत बाजू तपासली जाते. या तपासणीला DRE (Digital Rectal Examination) असे म्हणतात.

कोलोनोस्कोपी:

काही वेळा कर्करोग किंवा मोठ्या आतड्याचा इतर काही त्रास नाही याची खात्री करण्यासाठी डॉक्टर आपल्याला कोलोनोस्कोपी किंवा एन्डोस्कोपीचा सल्ला देतात.

मूळव्याधीचे प्रकार:गुदाशयाची तपासणी किंवा प्रोक्टोस्कोपीनंतर डॉक्टर रुग्णाला कोणत्या प्रकारचा मूळव्याध आहे सांगू शकतात.

• दुसरी पायरी – गुदद्वाराच्या अंतर्गत भागात आलेली थोडी मोठी सूज / कोंब. शौचाच्या वेळी ही सूज बाहेर येते.

आहारातील बदल व स्वतः घ्यायची काळजी –

आपल्याला होणारा मूळव्याधीचा त्रास बद्धकोष्ठामुळे असेल तर शौच नियमित व सुलभ होणे आवश्यक आहे. अशे केल्यास शौचाच्या वेळी जोर करावा लागणार नाही.

• यासाठी आहारातील तंतुमय पदार्थांचे प्रमाण हळूहळू वाढवावे – फळे, कोशिंबिरी, पालेभाज्या, सुकामेवा, कडधान्ये इ. पदार्थ तंतुमय पदार्थांचे स्रोत आहेत.

ही शस्त्रक्रिया पारंपरिक शस्त्रक्रियेला पर्याय म्हणून वापरली जाते. ही शस्त्रक्रिया संपूर्ण भूल देऊन केली जाते. गुदद्वाराबाहेर लटकणाऱ्या मूळव्याधीच्या कोंबांसाठी देखील ही शस्त्रक्रिया करतात. बऱ्याच रुग्णांना या शस्त्रक्रियेनंतर पुन्हा दुसरा मूळव्याधीचा कोंब बाहेर लटकण्याचा त्रास होऊ शकतो. पारंपरिक शस्त्रक्रियेमध्ये हे प्रमाण कमी दिसते. इतर उपचारपद्धतींपेक्षा या शस्त्रक्रियेत गुंतागुंत होण्याची शक्यता असते. त्यामुळे ही उपचार पद्धत पूर्वी पेक्षा कमी प्रचलित आहे. काही वेळा स्त्रियांमध्ये गुदाशय व योनीमार्ग( fistula to vagina ) जोडले जाणे किंवा गुदाशयाला भोक(rectal perforation) पडणे यासारखी गंभीर गुंतागुंत झालेली देखील दिसून येते.

• मूळव्याधीच्या शस्त्रक्रियेनंतर गंभीर गुंतागुंत होण्याची शक्यता खूप कमी असली तरी काही धोके संभवू शकतात –

फिशर म्हणजे काय(What is a fissure)?

फिशर म्हणजे गुदद्वाराच्या खालच्या बाजूला त्वचेजवळ जखम भेग पडणे. कठीण शौच असेल तर गुदाशयाच्या आतील नाजूक आवरणावर दाब पडून किंवा पातळ शौचाच्या वेळी जोर केल्यामुळे फिशर होते.

फिशरची लक्षणे कोणती आहेत(What are the symptoms of Fisher)?

वेदना: रुग्णाला शौचाच्या वेळी व शौचानंतर वेदना होऊ शकतात. फिशरच्या वेदना अतिशय तीव्र असतात. या वेदना काही मिनिटे ते काही तास असू शकतात. गुदद्वाराच्या भोवती गोलाकार असलेले स्नायू आकुंचन पावल्यामुळे या वेदना होत असतात. या वेदना इतक्या तीव्र असतात की रुग्णाच्या शौचाला जाण्याची भीती वाटते आणि त्यामुळे बरेच रुग्ण शौचाला जाण्याचे टाळतात. परंतु यामुळे constipution होऊन शौच कठीण होते व आणखी वेदना होतात.

रक्तस्त्राव: शौचाच्या वेळी त्या जखमेतून रक्तस्त्राव होतो.

काही वेळा फिशरजवळ किंवा त्याभोवती सूज येऊन अतिरिक्त त्वचा तयार होते व लटकू लागते. बरेचदा यातून चिकट स्त्राव येतो आणि त्यामुळे त्वचा खरचटल्यासारखी होते व खाजते.

शस्त्रक्रियेविना उपचार(Non-surgical treatment in marathi):
